I have included a dot-file for Winword that simplifies the creation and preparation of Word- or RTF-Documents. I have inserted most of the paragraph formats that are recognized by rtftohtml and bound keyboard shortcuts to them. Two Macros are provided for marking selected text as a cross reference (Control-Shift R) or as literal HTML (Control-Shift-H).
Perhaps this helps a bit.
I had some problems with index-entries that are inserted by an older version of MS Word for the Macintosh (5.0 I think). Try to use a newer one (Winword 2.0 works just fine on PCs).
